    If you read the notes and it has assessment, dx, hx, etc, then you have a diagnosis. I only know of the VA using a problems list, even then I have diagnosis that isnt in my problems list but its all in my records. Additionally my ENT doctors kept a separate list from my primary doctors. They are probably writing down diagnoses. Youre just wanting it in a specific format. Its not needed. As long as they write it down and youre seeking treatment/medications, etc. to manage symptoms youre fine.

    I wish I new I have been treated and taking meds for insomnia from my psychologists at the va hospital and they won’t add it to the problems list. My neurologist diagnosed me with chronic migraine and vertigo In her notes but I can’t have it added to the problem list. My chest CT showed emphysema, pulmonary doctor had me take a pft test diagnosed me with emphysema,copd can’t have that listed to my problem list. That’s all been in the last year and orthopedic diagnosed me with Patellofemoral syndrome in 2014 and I can’t have it added to problems list. Va hospital is very odd lol, yes I have some of this stuff service connected from the diagnosis in va notes but there’s a few they won’t service connect because it doesn’t show in problems list like my insomnia I filed they told me I didn’t have a diagnosis cuz it wasn’t on problems list but notes say veteran has a diagnosis of insomnia and the treated by w.e medication they are trying at that time.
