    At what point did the insurance seller provide any documentation that it paid the vendor $*n*, where $*n* is its contractual obligation to pay the vendor for <*something_here*>?

    Find everyone high up at the imaging company and send them a brief recap of what’s going on. To every possible address you can find.

    Ok so most of the time they go go soft collections before a true collection, but you def need to try to contact a supervisor at the medical facility and let them know what’s going on. I would also try to talk to a supervisor at your insurance and let them know you were sent to collections. It’s not legal to send a patient to collections for a balance that was from something other than a copay , coinsurance, or deductible . Your medical office needs to figure it out or write it off .

    Seriously, for a $15K bill, spend the $250 it will take to hire a lawyer to send a letter. That will stop the collection in its tracks. Give the lawyer copies of everything (all your correspondence, etc.). It’s a semi-expensive answer, but it will end the nightmare. Just make sure you interview a few lawyers before you hire one.
