At a minimum they should be able to answer your questions or get you help over the phone.
A medical record? Yes. Any other law enforcement indexed record in some sort of policing database? No. A Criminal Record? Absolutely not.
It will go on your youth record but once you become an adult, you can apply for your youth records to be sealed or destroyed. You can restrict access to your medical records but you would need to inform each medical provider of the restrictions, it might be sill possible for your parents to access your medical records. Look for help locally and get them to sign a confidentiality and non-access form while you are seeking treatment. Don’t use a hospital to seek treatment unless you have physically harm yourself.
I’m not 100% positive but the ‘consequences’ would be more for insurance policies and potentially things like military, police etc where if down the road you were wanting to gain employment that might be a barrier or a reason for not qualifying now I’m not 100% positive because I believe it depends on how the hospital would diagnose you and the individuals assessing your application.
But other than your medical records there isn’t a record anyone can look up or anything. There is confidentiality so the hospital can’t just call up your school and say so and so I hear because of… however there are some resources in the hospital that you may get referred to for support for in community and schools generally know who those people are and who they work with now that doesn’t mean their clients are only people who have attempted generally more just people who need extra supports weather it’s because of trauma, eating disorders, suicide attempts etc and they have a confidentiality agreement with you so they can’t tell the school your medical history unless you allow them too. Honestly working in schools and mental health it’s unfortunately way too common for youth to be in the same place as it seems like you are and genuinely everyone will want to help as best they can (that doesn’t mean everyone is perfect or not limited by bureaucracy). Being said if hospitals aren’t really helpful look at local organizations that offer in community counseling for free they are a good source of finding more resources if they are not able to help.
All in all it really doesn’t affect you down the line and none of that matters if your no longer here anyways so don’t let the faint possibility of some small consequence take away an opportunity to get help.

You will not have a criminal record. If police are called and have to bring you in, you will have a police record of contact. It’s not a big deal. You can later apply to have that destroyed.
It will be entered in your medical record. Medical records can be accessed by most doctors and nurses who you see in the future. Applying for certain jobs will also need a waiver to access your medical records and a suicide attempt may preclude you from doing certain ‘high risk’ jobs in the future (airline pilot, RCMP officer things like that).
They would need to inform your legal guardian you are in the hospital. As a minor, the requirement to inform your guardian ‘why’ isn’t absolute, but they would likely need to inform an adult who is responsible for you as part of a safety plan to ensure that you continue to be monitored any treated after your release from hospital. I would say your parent will be informed. Additionally, if there is concern about your parents ability to keep you safe, or if your parents are the cause of your suicide attempt or knew and did not seek proper treatment, they may also inform child service. They will not inform your school, though your parents may in an attempt to get you additional help or supervision at school.
