If you make a statutory declaration stating that you’re not aware of any similar infringements in your name in the past 5 years, this is still an accurate statement
It would be a false statement, it is very rarely worth putting a lie in writing, even more so an official document where you swear to the lie.
Here in NSW, the waiver is for points related fines and not for parking fines. Is that the same there in ACT?
The answer is clear: Did YOU commit the previous infringment, and are YOU signing the stat dec to say you didn’t?

You paid a fine in someone else’s name. Therefore you have not incurred any fine UNDER YOUR NAME in the last 5 years. So you could sign a stat dec to that effect.
Nobody in official status knows it was you driving at the time. The fine was issued to the registered owner (your relative)

Is it a legitimate fine from the Act Gov? Some Wilson carparks issue one that looks like a fine but is not. So depends all on where you were parked when you got the ticket.
