    This sounds like your former employer has entered into bankruptcy proceedings and has identified you as a person potentially owed some of the companies assets. If they found that some people are owed wages, it’s possible they’ve sent letters like that to every single employee they had for the past few years. Not super uncommon in bankruptcy proceedings.

    Still can’t give you advice without knowing exactly what letter entails, but with the info you provided, it doesn’t seem like you’re being sued or anything like that.

    EDIT: I read your post again and realized I missed that it was an Opt Out packet. Yep, almost positive they are entering bankruptcy, sent you that in case you are owed wages and are offering/asking you to opt out of the bankruptcy process if you don’t have a claim.

    No need to sign anything though. Opt out forms are for their convenience.
