Did the voice an actually state “I’m a lawyer”? Misrepresentation is a big deal. I’d hold onto that voicemail in the very unlikely chance you’ll need it later.

Ignore the call. If you are sued, deal with it then. If your review is opinion and not anything you’re stating as fact that is untrue, they likely won’t be successful in a defamation lawsuit should they attempt one.

I am an attorney based in Oregon and I’ve done a lot of civil work on both sides. Defamation is notoriously difficult to prove in court and no lawyer with any sense is going to file a defamation suit based on a review. The doctor is trying to intimidate you into silence and you should report him.
Contact the office of the attorney that contacted you, and ask if they are currently representing the doctor. If they say yes, just say thank you and hang up. If they say no, I bet they would love to hear that someone is impersonating them.
How did they get your phone number? It’s very much illegal to use information in a patient’s chart for any reason outside of medical care. Like say, giving it to a lawyer to make a threatening phone call.
Did you leave a review for an office you didn’t go to? If so, i would erase that one.
