    Who are you doing the signed affidavit for?? Employer? Police check? I think you are best to go back to them and ask for advice.

    Ordinarily I’d say something like a Statutory Declaration of all the names you have been known as might suffice, but it really depends who you are proving your identity to. You may have to seek advice from your home consulate to prove your identity. (Defence/security clearances, Government Depts like ATO or Centrelink or Medicare, working with vulnerable population checks, banking and Telcos (“know your customer“ legislation) may have a higher bar to get over than some other places)
