The ED to Home program is a new addition to UC Davis Health’s suite of in-home care services, which began with the successful implementation of the Urgent Medical Care At-Home program just last year. These initiatives aim to enhance patient access to care, improve the patient experience, and reduce unnecessary return visits to the emergency department. UC Davis Medical Center is one of the few hospitals nationwide to implement a program like ED to Home.

Colby, medical director of emergency medicine, not only benefits patients but also helps increase hospital bed capacity, potentially reducing wait times and freeing up resources for more complex cases. DispatchHealth will operate two vehicles within a 25-mile radius of UC Davis Medical Center, delivering care provided by a nurse practitioner or physician assistant, along with a trained medical technician.

The program’s services will be available from 8 a.m. to 10 p.m., 365 days a year, offering care for six to eight patients per day. Kelly P. Owen, professor of emergency medicine, highlighted the benefits of receiving care at home, including a lower risk of falls, infections, and improved outcomes. Patients requiring ED to Home care services will be referred upon discharge from the emergency department or by their primary care provider.

BJ Lagunday, executive director of Ambulatory Operations and Population Health, expressed enthusiasm for the collaboration, stating that bringing medical care directly to patients’ homes is a significant advancement for both the health care system and patients. The ED to Home program is part of UC Davis Health’s Connected Care Center, which aims to deliver a range of services directly to patients, including Express Care and Urgent Care at Home.

Key collaborators in the program include Michael Condrin, interim chief administrator of UC Davis Medical Center, Bruce Hall, chief clinical officer for UC Davis Health, and Rupinder Sandhu, nurse director for Emergency Medicine.
