An in-depth exploration of this remarkable evolution has been compiled in an extensive 800-page book titled A New History of Cataract Surgery, authored by Dr. Chris Leffler, an associate professor in the Department of Ophthalmology at Virginia Commonwealth University School of Medicine. This text is the latest addition to a historical series from Kugler Publications that focuses on the field of ophthalmology. Dr. Leffler, along with a team of 43 co-authors, which includes faculty members, residents, and medical students, dedicated years of meticulous research. Their efforts involved scouring medical records, exploring historical archives, visiting libraries throughout the nation, and engaging with experts from around the globe.

Dr. Leffler expressed his enthusiasm for the project, stating, “There’s a long chain of people through history, for several thousand years, who have been doing this surgery in one form or another.” He emphasized the joy of piecing together the intricate historical narrative of cataract surgery and how it all intertwines.

Mary Daly, M.D., chair of the Department of Ophthalmology at VCU, praised Dr. Leffler and his team for their contributions to the field. She highlighted the profound impact their dedication has on the department and its students. “Their love for our field is so strong that it permeates through the department and extends to our learners in an incredibly impactful way," Daly remarked. "The VCU faculty, residents, and medical students who contributed to this text are all active participants in the living history of ophthalmology — not just through their beautiful retelling of the timeline of cataract surgery, but through their real-time, day in and day out efforts to improve the vision health of each and every patient they meet."

In-Depth Analysis of Cataract Surgery’s History

The two-volume work provides a comprehensive account of cataract surgery’s journey from its inception in antiquity to contemporary advancements. Among the significant milestones included are the first documented removal of cloudy lenses in the 1750s, as discovered from a previously unidentified log by a French ophthalmologist in 2004; the introduction of the first intraocular lens (IOL) implant in 1950 at a hospital in London; and the innovation of silicone foldable IOLs during the 1970s, which allowed for smaller surgical incisions and transformed recovery protocols. Today, patients have a myriad of IOL options available to them, some designed to eliminate the necessity for reading glasses entirely, although as Dr. Leffler acknowledged, “the quest for perfect correction of presbyopia is still ongoing.”

With the remarkable innovations and enhancements in cataract procedures over the last fifty years, there remains a wealth of knowledge among the pioneers of these advancements, many of whom are still alive and able to share their insights. In the past, patients undergoing cataract surgery often faced prolonged hospital stays and were required to remain still with their heads supported by sandbags during recovery. Dr. Leffler and his extensive co-author team aimed to capture these personal anecdotes, ensuring that these invaluable experiences were documented for future generations.

“There are just a lot of really great stories, but some of these individuals are older, so this is our last opportunity to truly capture their recollections," he observed.

One of the contributors, Dr. Stephen Schwartz, who served as the program director of VCU’s ophthalmology residency from 2002 to 2004 and is currently a professor of clinical ophthalmology at the University of Miami Health System, lauded Dr. Leffler for spearheading this monumental project. “This is probably the new standard on the history of cataract surgery, and I’m happy to have been a small part of it,” Schwartz noted. He described the book as an outstanding achievement for Dr. Leffler and an important milestone for VCU’s reputation in the medical community.

For Dr. Schwartz, the extensive hours devoted to researching and writing five chapters were invaluable. He believes that grasping these historical narratives fosters a deeper understanding and reverence for the field. “It’s not that knowing this history will necessarily make you a better doctor or help you take care of patients,” he shared. “But I do think it gives you a more complete appreciation for how we got here and the rationale behind many of our practices today.”
